“On the day” telephone appointments provide access to your choice of health professional (your usual or choice of GP, advanced nurse practitioner, mental health practitioner)with a face to face consultation arranged as necessary and agreed together. We encourage patients to contact their usual GP. A list of GP working days are on the website under the Appointments tab at the top of the page.

 

 

What Happens On The Day?

On the day telephone consultations are available between 8.30- 6.30pm Monday to Friday. On the day you wish to seek advice, please telephone the surgery on 01225 896630 and choose Option 1, to speak to a care navigator, who will direct you to the appropriate resource or offer you a telephone appointment on the same day with a health professional. The telephone appointment may be sufficient to deal with your enquiry, or you may be asked to attend the surgery later that day for an appointment to investigate your problem and possibly for tests.

Appointments with a nurse or health care assistant for routine healthreviews and checks, blood tests and other investigations are booked in advance.

Patient Check In System - When you arrive for your appointment - please check in using one of the touch screens to the right in the foyer. This allows the doctor or nurse to know you are waiting to see them.

Patient Call System - The practice also has a patient call system that the doctor or nurse will use to call you to their room for your appointment. There are two screens, one at each end of the waiting room – please keep checking the screen because it will inform you when the doctor or nurse is ready to see you, and which room to go to

 

The benefits for patients include:

  • Access to medical advice on the day and rapid resolution of medical problems - We also offer on line appointments for direct booking with a GP or nurse until 7.30pm on selected weekdays. Since 1st October 2018 we are part of a pilot to offer ‘Improved Access’. Your clinician can book you into appointments , until 8pm on weekdays and from 8am-1pm on Saturdays.
  • Choice of health care practitioner -We offer a range of specialised clinicians including doctors, advanced nurse practitioners, mental health professionals,  a contraceptive and menopause specialist, a diabetic specialist and  a well established ‘care of older people’ team and nursing home clinicians. We encourage continuity with your usual doctor or practitioner during an episode of care, as this improves medical outcomes and satisfaction.
  • Quality  We try to make visits to the surgery as productive as possible, with blood tests and investigations on site on the day, if available. Clinicians have access to advice from secondary care consultants via direct phone links, which has improved primary and secondary care interaction.
  • Community integration the MOPT (Melksham Older Peoples' Team)and Nursing Home Team, operate from Giffords Surgery and offer a range of medical services in house and in the community for our older patients. They liaise with social services and district nursing teams providing a highly acclaimed service

You can now see a GP in the evenings and at weekends.

Wiltshire patients can now arrange to be seen 18:30-20:00 Monday-Wednesday Friday, Saturdays, Sundays and bank holidays. Practices across West Wiltshire are working together to provide this additional capacity. For Giffords  patients: Monday to Wednesday evenings will be provided at Giffords Surgery, with Friday evening and Saturday mornings being provided from one of three hubs in rotation (Giffords Surgery , Melksham, Trowbridge Health Centre or Westbury Group Practice. Sunday mornings will be provided from Southbroom Surgery in Devizes. Giffords Surgery is also looking after Spa Surgery patients.

To book one of these appointments, please contact your registered Surgery during normal opening hours. A small number of appointments will be available via 111 outside of normal hours.You will need to give consent for your medical information to be shared between providers to use this service.

OUT OF HOURS

Between 6.30pm and 8.00am Monday to Friday, weekends and Bank Holidays, an out of hours service is provided by Medvivo. Please telephone 111 for this service.

You can call 111when you need medical help fast, but it is not a 999 emergency.NHS111 is available 24 hours a day, 365 days a year. Calls are free from landlines and mobiles.


TEXT SERVICES

Giffords offers an automatic text appointment reminder service. Toregister for this service please complete a form at reception.

In addition to automatic appointment reminders, health professionalsmay text you with advice in response to investigations, for example,asking you to make a telephone appointment. This recent innovation is appreciated by patients and improves our communication with you.

This proactive response to results is in addition to the telephone results line open daily between 2-4pm, for you to access your results. While we endeavour to contact a patient with an abnormal result, it remains your responsibility to follow up tests with an enquiryfor the results.

You can register for online access to your results through our website by making an enquiry at reception.

We deliver health information via the smart phone app ‘MJOGMessenger’. Information on how to download this is on our website, displayed at the surgery or available at reception
